    The North Dakota Veterans Home is seeking a compassionate, organized, and dedicated Social Worker to join our team. In this role, you will support veterans and residents by promoting their highest practical physical, mental, and psychosocial well-being while helping ensure dignity, choice, and quality of life throughout their stay.

    This position works closely with residents, families, interdisciplinary teams, and community partners to provide supportive counseling, care planning, advocacy, crisis intervention, discharge planning, and coordination of services.

    Located in Lisbon, North Dakota, the North Dakota Veterans Home is a long-term care community dedicated to honoring and serving our nation’s Veterans and eligible family members. The home includes two basic care pods and one skilled care pod. Our team provides high-quality, compassionate care in a respectful, team-oriented environment that values dignity, purpose, and connection.     Duties and Tasks

    Provide supportive counseling and emotional support to residents and families

    Assist with mental health assessments, behavior management, and crisis intervention

    Complete psychosocial assessments, social histories, and required documentation

    Participate in interdisciplinary care planning and resident care conferences

    Support resident rights, advocacy, dignity, and individualized care preferences

    Assist with admissions, orientation, transfers, and discharge planning

    Coordinate community services, referrals, transportation, and outside resources

    Help residents access financial benefits and support programs including Medicaid, Medicare, Social Security, and VA benefits

    Collaborate with physicians, psychologists, nursing staff, and other care team members

    Maintain accurate documentation and ensure compliance with state and federal regulations

    Minimum Qualifications

    • Bachelor’s degree in Social Work
    • Licensed Social Worker (or ability to obtain licensure if required by state guidelines) preferred
    • Strong communication and interpersonal skills
    • Ability to work compassionately with aging adults, veterans, and families
    • Strong organizational and documentation skills
    • Ability to work collaboratively within an interdisciplinary team
    • Strong computer skills, including proficiency with electronic health records (EHR) and Microsoft Office applications
    • Previous social work or healthcare experience preferred
    • Knowledge of long-term care regulations and resident rights preferred
    • Experience with crisis intervention, counseling, or behavioral health preferred
